Help needed to get I-693 forms

Hi my I-140 recently got approved and i am new to this I-485 board. I scheduled an appointment with doctor next Friday (09/07/2001), but they don\'t have the I-693 forms. I work in NY and live in New jersey. Which is easiest way to get the forms. I spoke to my lawyer she said she is trying to get the forms. Any help is appreciated.

Try the ins website...

they have a bunch of forms there.

Check with some INS approved physicians. They may have the forms. My doctor gave me the forms.
