Help needed in filling I-130 form

Hi,

I am filling the form to sponsor my mother. She is a widow. what should i fill in "number 11. name(s) of prior husbands/wife" leave it blank because she is a widow?

Also number 17 "list husband/wife and all children of your relative" should I put my name also as I am her daughter?

I would really appreciate any help.

Fill in all the information asked for. #6 will tell them she is a widow.
