Help needed for H1B extension

In order to save $$$$, I have the 1st application copy and want to file my H1B extension by myself. To my understanding, the extension application package should include:
(i) petition letter from employer;
(ii) LCA;
(iii) I-129 & I-129W;
(iv) copy of I-94;
(v) $185 paycheck to USCIS.

My questions are
(1) Are these documents enough for 2nd 3-years H1B status extension? If not enough, what's the extra do I need to file?
(2) The current 129W still requires $1000 filling fee. As I know, this fee is canceled sometime last year. $185 is all the fees I need to pay, if I don't do Premium Processing. Am I correct? Then, how should I fill in 129W Part B? Can I check all to "No", or leave them blank?

Hi please find following list which i sent to USCIS. FYI, I did apply through web-site using E-filing process.

1. Confirmation Receipt for E-Filed I-129.
2. Petition for Extension of H-1B1
3. E-Filed Form ETA 9035E.
4. Copy of last 3 pay stubs
5. Copy of W2 Statement
6. Copy of Foreign Academic Credentials Equivalency Evaluation
7. Copy of Degrees and Transcripts
8. Copy of I-94
9. Copy of valid passport
10. Copy of All previously approved I-129 including current one.

regular fee is only $185, no need to pay additional $1000.
if you want premium processing (which i will suggest to do) then only need to pay $1000.
