Help needed for 485 Required documents

Hi,

I am in the I-140 Stage and was preparing the documents for I-485.
I have few questions regarding the documents, would really appreciate
if someone can help
1) I have birth certificate but with my first name and my mom\'s name is also not full, do we require all full names??
2) On my marriage certificate is it necessary to have parents full name and the full address of the place of marriage (with state and country)
3) Do we require an affidavit to support the change of name of my wife.
Right now we have some government certificate from India.
Does anybody has sample affidavit for birth, marriage and change of name??

Affidavits...

For all of them you can provide affidavits.
1) BC :- 2 affidavits from parents/ elder relative citing all the different names.
2) As long as it is a govt. issued marriage certificate you are fine. You can get another affidavit if you want from the person who conducted the ceremony or a local magistrate
3) If the name was changed, your spouse\'s passport should be modified. That should be enough to get the GC in the desired new name. Just fill all the possible other names she had before in the 485 app (other names field)
immihelp.com has the affidavit format. Just modify that to suit marriage or other needs.
