Help in filling N-600 form

I am naturalized citizen and was trying to fill up N-600 for my son.

In Part 3 under 5A (Information about entry into US and current immigration status) , it is asking "I arrived in following manner , Port of Entry, Date of Entry and Exact name used at time of entry"

Is this for last time he entered the country or very first time.
My son came here 10 years back on H4 and than got Green Card.

Any help will be appreicated.
