Help: How to Change of Status from H1B1 to H4?

Currently, my spouse has H1B1, and I am also H1B1. I want to change my status to H4 since I will go back to school. I have some questions.

1: What documents should I mail to USCIS?

2: In form I-539, Part 3.3 - Is this application based on a separate petition or application to give your spouse, child or parent an extension or change of status? Which one should I select (NO or YES)?

3: When will be the last day of my H1B1? and Once I get the first Notice, not final approval notice, Can I go to school for study?

------------ form I-539, USCIS fees, copy of your valid I-94 front and back, Copy of spouse H1 approval and spouse valid I-94 front and back, your passport pages with visa stamp pages

As to second question:
I do not know how to answer the Part 3.3. whic one I should choose :NO or Yes .
As to third question:
I am wondering when I can tell my company that I will quit my job, and can I go to school before I get approval.
