Help - Documents - Toronto stamping

Me and mywife are planning to go to Canada for dual purpose of getting Canadian PR and US visa revalidation (Toronto Consulate).

My first stamping(H1B) was from India, which expired in May'2005. Meanwhile I obtained MBA (while working on H1B)from US university and joined a new company (based on MBA, transferred my H1B) in Jun'06.

I need some help on documentation;

Do I need to fill DS-156, 157 for me only or for my wife also?
I filled in DS-156 forms online at evisaforms.state.gov, where can I get DS-157 form?
Also when I completed DS-156 form, barcode on the first page and the third page doesn't match. is it ok? please share your experience.
Do I need to pay fees(USD100) for me only or for my wife also?
Can I ask somebody to pay fee on behalf of me, well in advance? In that case what are the documents required?

Latest I-797A with I-94 for me and my wife
Copy of I-129 for me
Employment letter from my current employer (Does anybody have the sample content for the letter?)
Paystubs (last-4) from my present employer
Bachelor Degree (India) - All marksheets, transcript, degree evaluation
Masters Degree (USA) - Degree Certificate, transcript
W2 for last three years

----- You have fill out the forms both for yourself and your wife

----- The bar codes are generated by the system, you will have nothing to do it. Leave them as it is

Yes, you need to pay for yourself and wife (USD 100 each)

Can I ask somebody to pay fee on behalf of me, well in advance? In that case what are the documents required?

----- Yes, someone can pay on your behalf and I believe the documents needed are the appointment letter. You can also check with Scotia Bank by calling their 800 number

----- Carry the following, just in case
- Proof of residence: Utility/Telephone bill
- Employment letter - Each employment, Employer and Employee are different, so carry a version that you make up or if your company has a format then take that
- Keep a photocopy of all your documents
- After getting stamped take a photocopy of your visa stamp and keep it with you
