Help! DATE on I-765

I will file I-765 with my I-485. On I-765, there is column 11:
Have you ever before applied for employment authorization from INS?
Which INS office?
Date(s).
Do you know which date this "Date(s)" means? Is it the date when I applied for my previous EAD? If so, is it the date I dated (after my signature) on my previous EAD I-765 form? Thanks.

Just Put

Just put in your last EAD starting date.

If this is your first EAD, put in your H1-B date

If this is your first EAD, just tick \'NO\' but not H1-B date
