Help: AP renewal

Hi,
I wanted to save some $$ applying for AP renewal.

I downloaded the Form I-131 from the www of BCIS at
http://www.immigration.gov/graphics/formsfee/forms/i-131.htm

1. Is this the right form ??

could u pl help me with
2. docs we need to send with the application ??
3. Fees (it says $110 on the www BCIS and the instructions page says $65.

Part 1.
4. A# Is this the A# on my previous EAD ?? (My 485 is till pending)

Part2

5. Application type
check which box ?

my 485 is employment based.

Part3

6. Date of inteneded departure
does this need to be exact date or can we say in months or can we give and approximate date ?

(in this same section)
we received an approved AP that is going to expire shortly. Do we need to mention here, although the form asks us to SKIP.

Part 7
on saparate sheet of paper, please explain how you qualify for an Advance Parole and what circumstances warrant issuance of AP. Include copies of any documents you wish considered.

AP Renewal Question

I hv my I485 pending at VSC
Got my First AP and EAD done by the Attorney

Now it is renewal time and I want to know if I have to send my I131 (AP Form) to Vermont or Nebraska ?

I am confused after I took a look at the following on BCIS WWW site.

All Forms I-730, Refugee/Asylee Relative petitions are filed with the Nebraska Service Center.
All Forms I-485, Refugee/Asylee/Haitian applications are filed with the Nebraska Service Center.
