hello guys,

I am a DV-2008 winner, I would like to know what documents should i fill out? I knew that i only had to fill out I-485 for adjustment of my status, and in this forum i saw the people filled out some other forms.

Also another one question, when i received the second letter from KCC that says my case is now throught USCIS, i didn t receive anything for medical examinations. I think i saw somewhere in this forum someone who said that he received forms for the medical examinations but i didn t receive any forms.

P.S on the first package i received for the ds forms in the instructions they say the want some documents of proof, like brith certificate etc but they don t say anything about medilac examinations.

I am on OPT now.
 
Ruxy i read the thread before i posted this, by reading that i got confused..
question 1:
did you get any forms for the medical examinations?

question 2:
except I485 what other forms do i have to submit? (where can i find that? because when i called uscis, the operator told my that i have to fill out only I485)

you do not need forms for medical exam. The USCIS civil surgeon will have them. You will have to submit the sealed package you get from the civil surgeon with your AOS package.

You had to fill I-485 with USCIS and DSP-122 with DOS. I-485 has some supporting forms like G-325A, that you may have needed to fill out. Read the instructions to I-485. You may not need to fill out I-765 or I-131. You do not need I-864.

When am i gonna get the AOS package? when i send the 375$ (dv fee)? or after i send the I-485 in ?
 
you are not getting anything.

When your number becomes current, you will download all the forms from the link I gave you above, and you willl send the AOS package to USCIS.
