Health/disease screening in addition of health/travel insurance

Hi Friends,

I have fresh H1B stamped. Now i want to travel to USA from india.
- Where disease screening is done. I mean at port of entry or what place.
- Can you please tell what are health, medical or disease related issues or steps.
- What about medical/travel insurance. When it should be done.
- Any other issues i may face during travel from here to destination, which are mostly not documented or not well known.
