Dear Hadron,
I am almost at the end of j1waiver, and I have an approved NIW for my current service. I have good job offer in another state, which is MUA (but not HPSA).

I have filed new NIW I-140 for prospective employment. I was able to get state DOH public interest attestation letter for my application. It seems like it won't be adjudicated in near future, and I am not sure if MUA but no HPSA designation will pose any problem? I would like to know your opinion. Thanks!

Don't think the feds care. It has to be an area 'designated by the secretary of HHS determined blablabla'. As long as you have the DPH support letter for the new location, you should be fine. To my understanding, your time served in a different hpsa/mua/state during your waiver gets credited, I am actually not sure whether you had to file a new I140 or whether there is a way to just amend the old one.

Thank you for the help! There is no option for "amended I-140" in the form. Besides all the evidence has to be resubmitted for relocation, with a letter from DOH. Its like doing it all over again.

Do you have an attorney who has done this before ? I wouldn't expect that there is a particular form for it. This is where an experienced office can be of benefit.
