H4 Visa Stamping at Mumbai Consulate or Any

Hi!
Can any one tell me what documents required for H4-Stamping in mumbai consulate?. I am sure doc requirement is same for all consulate.

She already has H4 Visa stamped but it expired on FEB 5, 2005 , while she in on Vacation to india.I am in USA . She need to get her H4 stamp to re-enter in USA.

Also tell me , who much is waiting for appointment?
