H4 visa stamp required to enter back in US from Canada?

I have H4 approved until Dec 2005. Currently H4 Visa is not stamped on the passport, I have couple of questions.

Does H4 visa stamp is required to enter back in US from Canadian border?

At the time of entering to Canada, can Canadian visitor visa be applied and stamped at Michigan/Windsor border?

Any insight would be appreciated.
