H4 visa - need help with ds156 form

I'm getting married in December. To get an appointment in Mumbai for h4 visa, my wife needs to fill out the DS 156 form. The question is what should be filled out for question Marital status. I would really appreciate if anyone help with this. Also if anyonw could share their recent experience at mumbai counsualte
 
Married??

Only after getting a marriage certificate will she be able to submit the forms, right. So she should mention married in the DS156.
For appointment also, mention that she is married.
BTW, do you know Mumbai consulate insists on name change in the passport for the spouse?
