H4 Visa, Last/First name swapped

My wife is here in USA on a H4 visa since march 2004. In her USA Visa and I94 Form , the first name and last name are swapped; due to this her names look different in visa and her passport;
Her Name in Passport is as follows
surName Seena John - (here in the surname, sur is stuck out and only name is visible)
Given Name - Chaalattu (there is a "-" between john and chaalattu)
But in her USa Visa her name is as :
Given Name : Chaalattu
Surname : Seena John
She filled up the I-94 as per her visa where it was interchanged,due to this
interchange DMV is not accepting her Application and unable to apply for any thing like further> studies etc. how to correct this issue, since i am married is change of name
due to marriage an option here
