H4 visa Auto-denial

I am here on a valid H1-b and my wife is on a dependent H4 visa based on my H1 visa. My new H1 visa with my current employer was approved
in Jan 2006 and it is valid until 2/17/2008. I filed for my wife's H4 extension back in July 2006 at Southern Service Center (Mesquite, TX)
with all the supporting information. Her original H4 was valid until July 2006 and I filed her H4 around 1 week prior to her previous H4 getting expired.

Since then, the status is showing as "Request for additional information" and even after repeatedly calling the National Contact Center (800-375-5283), we were not able to receive the letter from USCIS regarding what additional information is missing.
Though, we did receive the letters regarding our call records to the customer service.

Just yesterday, I recieved a H4 denial in mail because 90 days have been passed and no response was sent to USCIS, the irony is that we did not even know what response we had to send.

My questions:
1) Can I write to USCIS explaining the issue and asking them to send me the RFE?
2) My wife just left for India last week, can she apply for H4 visa there at US Consulate? If so, do we have to mention the whole thing?
3) Will the 180 days time duration start from the day her original H4 got expired or the denial notice date (9th May)
4) Can we self represent the Motion to Reconsider the case? Though in this situation, I have all the supporting evidence that I tried multiple times to let USCIS know that I have not recieved the RFE.
