H4 status. How to get a tax-id?

Sorry, if I'm posting to wrong group.

I'm kinda new to this and any help is greatly appreciated.

I filed for tax extension this year. I need to file taxes by Aug 15th. My spouse has a H4 visa. My tax consultant says that I need to get a "Tax Id" for my spouse. Does anyone know how to get one? Ok, here is the deal: My spouse is in India and haven't yet been to USA (i.e There is H4 visa stamping but no I-94 etc)

Any help will be greatly appreciated.

In that form, line 6d, it asks for "Entry date in US". My spouse does not have that information yet. Any ideas whether that is needed?
