H4 questions....please help

Hi,
According to the US consulate the following is required for my wife\'s H4 visa :

1) Letter from my company ...stating what ?
2) I 797 original.........is this I 797 issued to me or seperately to my wife.

3) Would be a problem if my wife had a prior F1 visa and is changing to H4
now.

Please help

New H1b. Port of Entry Issues

I got my H1b stamped in my passport last year , but my employer is reluctant to call me , for reseaons you all know , yet they maintain my status.

Is it possible for me to come to the US without their approval , perhaps for two months? Would I get problems with the INS guts at the port of entry ?

I have a question :
 
Right now, she is in India, planning to come to US on April 17 2002 (Return ticket purchased). She already stamped Visa on her passport when I was in my old employer which is valid till Dec 1 2002. She was stayed in US for nine months during 2001. Now she has got H4 approval from New company with I-94.
 
Is she required to apply the visa for coming to US or she can make use of the Visa(which stamped on her passport)? if she needs to apply then they may ask my passport copy with visa stamping of my new employer which i have not yet updated.(?????) Just I sent it for stamping and it may take 3 to 4 months.
