H4 Extension for only 1.5 months

I filed for H4 extension for my wife. She got extension for only 1.5 months.
what could be the possible causes.
1) Mistake by USCIS?
2) Mistake by my Lawyer?

If its a mistake in Date bu USCIS then I assume that its is a safe position for us but What if the mistake has been made by our lawyer and he applied with wrong date?

Just to mention, myself and my wife are still very far from the 6 yr h1-h4 limit.

I will be very thnakful for any advise.

What is the expiry date of your current H1? Did you applied for H4 extension only or applied for both H1 & H4?

If you applied both together then the expiry date should be the same for both H1 & H4.
