H4 clause on Employment Letter!!

hi All! When applyin for H4 visa does ther has to be any speical mention/clause on the employment & wage status letter I get from my company which is required during application of dependent visa?

What I am looking for is a format of letter which I get from my company.

For now the company issues a letter stating:
Mr. X is an F/t Employee earning $X per annum.
.........................
.........................
what else I need from my company to include in the letter they give me when I apply for H4?
