h1b visa doubts

hi,

my brother got h1b visa from Jamaica on 2003. His initial stamping expires in 2006. I have some doubts regarding h1b renewal

1) can he renew the h1 b visa in US or does he has to go to india for it?

2) If he can renew the visa in US, then can he travel to india in future Or does he has to get the visa stamping from india for travel?

My brother's initial h1b visa stamping was done in Jamaica.

Renewal can be done in US and can stay and work in US, but visa stamping multiple entry should be done outside US.
2) If he can renew the visa in US, then can he travel to india in future Or does he has to get the visa stamping from india for travel?
He can go out of US but he has to get visa stamped from India to re-enter, if the initial visa in the pp is already expired
