H1B to GC in Underserved areas

Is it possible for a resident physician on H1B visa to file a GC in EB1 status by applying to the Underserved areas in US and commmiting for the period they need ?
Can you throw more light on how to go by this ?

How long does it take for labour certification for H1B to go thru EB2 NIW comapred to simple EB2 ?
