H1B from 98-03, now I want to work in US again

Hi,
I used to work in the US for company A on a H1B from 1998 until 2001, then started on a new H1B at company B.
Company B Chapter 11'ed in 2003, I got laid off and decided to move back to Germany. I have been working here since, but would like to apply again in the US.
My questions:
- Could I extend my (now expired) H1B I had with company A assuming I would be re-hired exactly there?
- Can I apply for a brand-new H1B once I found an employer willing to sponsor me (I read that one can do this
if one did live outside the US for at least a year)?
- Can I apply for a GC right ahead, providing I have a job offer (I know this is the H1B thread, just curious)?

I would appreciate any information.
