H1B Extn. / Visa Stamping- URGENT!!!

Can someone clarify the following questions:

1. For H1B Visa Extn (NOT the 6yrs), after the initial 3yrs period: Is there a rule for BCIS Center(Nebraska) that they will accept the Extn petition (I-539) ONLY 60 days before the existing Visa expiry date & NOT BEFORE THAT.

2. Once the Approval for Extn. is received is there a faster way of getting the new visa stamped on the Passport. I was informed that it takes 90days to get the new visa stamp from the Dept of State (DoS). I have this question as I had to visit another country other than my home country & also the fact that the Passport would be with DoS for these 90 days.

3. Reg. Extn Visa Stamping, the rule states that
"He/She could take the new approval notice to THE CONSULATE ABROAD WITH JURISDICTION OVER HIS FOREIGN RESIDENCE TO OBTAIN A NEW VISA"
Does it mean that
- I can go to only my home country to get Visa stamped OR
- I can get the visa stamped from Canada/ Mexico etc. OR
- I can get the Visa stamp from DoS / Home Country.

-- You can file 6 months advance 9 Extension petition for H1 is I-129 NOT I-539, I-539 is used for H4 extension)

----- It is taking around 90-100 days, no faster way to get Visa stamp in US
I have this question as I had to visit another country other than my home country & also the fact that the Passport would be with DoS for these 90 days.
--- You can ask back the passport if you have valid visa in PP
