h1b extension fee

Could someone help me please? I am filing for H1b extension, as my 3 years are coming up. Do I need to pay the $1,000 scholarship fee again, or can my employer be exempted? Is the process the same as for first time H1B?

No exemption

Process is the same. Fees are the same as new H1. Remind your employer to do the LCA before.

I filed the LCA and, even though I gave the same job desription, since I am still at the same position. But I was given a different DOT title than the one for my first filing.
Can this discrepency cause me any problem?
