h1b extension conditions

What are the conditions under which an H1 extension can be filed for?

Are they the following:

1) LC and I-140 approved – can file for 3 year extension

2) LC approved, but I-140 not yet filed or approved

3) LC filed and not yet adjudicated (365 days or more have passed since date of filing)



I am not sure about (2). Please clarify.

According to USCIS:
if your I-140 is approved *and* I-485 is filed then you can only get a 1 year extension.
But I don't think this is true in all the cases. Some of them have got three year extensions (includes me).
 
LC approved but less than 365 days?


Can I get an extension if my LC is approved but date of filing LC is less than 365 days?

Thanks great guru. Could you, or anyone else point me to the docs/webpages where the conditions for granting h1 extension are clearly mentioned..
