H1B Extension approval arriving after i-94 expiry

I have used my 6 years H1B visa allowance, applied for a Green Card and had 1 extension granted. Since then the application became lost in Philadelphia BEC, applied for re-construction, application is now "In Process" and may be used as evidence for 2nd extension. As a consequence the application for another extension has been seriosly delayed.

My question is: As my I-94 and current visa is likely to expire before receiving the approval for H1B extension, do I have to leave the US in accordance with date stamped on my I-94? If not, what are my options?
 
Thank you. I may have mislead you, my H1B application has been filed but based on current processing timescales, approval may occur after I-94 expiry. In this event do I have to leave the country and arrange for the approval notice to be sent overseas?
