H1B expired in May 2004, how to apply for extension

My H1B expires in May 2004 (1st 3 years). I have been out of the country once and have an I-94 stamped with H1 valid until May 14 2004. My questions are:

1) When should I file the extension?
2) Is it simple enough for my employer just fill the I129 form? Do I have to go back to ask the attorney who first filed it for me? I hired my own attorney the 1st time.
3) Do I need to get a new I-94 along with the extension?

----You can file H1 extension 6 months advance. Or before your current I-94 expires
2) is it simple enough for my employer just fill the I129 form? Do I have to go back to ask the attorney who first filed it for me? I hired my own attorney the 1st time.
----- Your employer has to get LCA from DOL that is attached with I-129, better go to lawyer to avoid any mistakes
3) Do I need to get a new I-94 along with the extension?

----- When the lawyer/employer gets LCA from DOL for next 3 years (one can get LCA in same day or in 2-3 days, it is fax back system) and employer asks 3 years on I-129 you will get new extended I-94 with H1 extension till the time asked on I-129 and validity of LCA. You need new extended I-94 with H1 approval notice or you have to go out of US after May 2004, get new H1 visa stamp then have to enter on H1 and get new I-94 at POE. If you have been in status you will get I-94 extended and attached with H1 extension approval notice.
