H1 visa Medical license in NY..need help

As many know, NY state has one of the worst requirements for unlimited license to practice medicine for H1 visa holders, I am in a situation where I finished my Waiver requirement for three years in the designated HPSA and got my I140 approved but hit with the Retrogression effect by 12 days for appying I140, I moved from my previous employer after fulfilling waiver requirement to a new Hospital which still qualifies for a limited license,
I just finished talking to Mr.Swanson , from DOH NY ( a very nice gentle man who is always helpful to J1 /H1 visa physicians in directing appropriately), as per Mr. Swanson, NY state will not issue an unlimited license to anybody who do not have a an actual Green Card, even if they have just EAD, this was kind of surprising.Can somebody give me some info on NY state extended license ( unlimited) so I can work in other hospitals.

This is how I understand NY licensing law:
You are in the same trap that many university employed O1 holders are in. You are important enough to get your 'extraordinary ability' or the 'national interest' certified, but NYS dept of education won't give you an unlimited license. It is my understanding that most people stuck in this predicament have to continue on these temporary 3 year permits which the dept. of education issues on a case by case basis. In all likelihood getting further extensions on this 3 year permit limits you to practice in underserved locations so the 'public interest' provision of the NY licensing law applies.

Could u please tell me if there is a provision for H1 visa holder ( After j1 waiver completion, hit with Retrogression and waiting for I 480, like me) to have a self sponsored second H1 visa to have my own practice thro'a PC in addition to my primary H1 visa which is sponsored by my current employer. In other words, I am planning to have my own practice in addition to my primary hospital based job ( I have completed my waiver), honestly , this choice I am looking primarily to get some tax benefits and have my Primary care practice in shape , any suggestions please

I don't think you can get sponsored for an H1 by a corporation that you own >5% interest in.

You can file for an NIW for one of your underserved locations and use the EAD to work at a non-underserved location. But that might be a problem from the angle that your 'special license' might be restricted to a particular practice location (I don't know that for a fact, as you already have a special license you probably know that better).
