H1 Visa Extension

Hi,

My H1 visa extension is getting expired in Oct 2005. I am planning to file for my visa shortly.
Please help me for the following questions:
1) This is my First three year H1B visa from 2002 to 2005 and I was out in India for about 11 months in 2004. (I am presently in US though). Will this effect for my visa extension.
2) My Indian address in my passport is my old residential address. Should I use my old address in passport for my visa extn or should I use my new address.

Please help.

No, If you are out of US , you can recapture those days at the end of 6 years.
