H1 to H4 Transfer (please advice very urgent)

Hi,
I am working In USA on H1-b Visa. My Wife is also working here on H1-b. I I have been asked to leave by March - end. What is the procedure to file for H4. What docs I need to submit, How much fees I need to submit, and Where I need to post all these docs. How much time it will take to get H4 Visa.

The service center with jurisdiction over your residence.

How much time it will take to get H4 Visa.

Normally 3 to 6 months but the answer depends on the service center.
