H1 to H4 and back to H1

Hi All,
Here's my situation:

I changed from F1 to H1B (effective 01-OCT-2004) but have not got it stamped. Now, if I file for an H4 without getting the H1B stamped, would my old H1B stand cancelled?

Also, if I choose to change from this H4 to H1B with a new employer, would I require paystubs from my old H1B?

Any help would be appreciated.
