H1 revalidation questions

On the travel.state.gov/revals.html page it is said what
documents are to be sent and address. But they did not
say anything about how to pay (check, money order?),
how to provide return envelope, etc. Is all this information
existent on the forms to submit for revalidation?

Who recently used this: how long does it take?

Also, if I apply for visa at consulate in my country I pay reciprocity
fee, but on that HTMP page they say I need to pay only $65 per
visa. Is this the case, or somewhere else it is mentioned that I
would have to submit this reciprocity fee?

Did you see http://travel.state.gov/hlop.html

If you use FEDEX, your credit card information is enough to take care of the return postage. If you are using USPS, just add the postage equivalent of the documents you are sending (Whatever is the total weight of the envelope before sealing it, should be the return postage affixed). Use common sense.

Sample Employment Letter

Can anyone suggest proper text and/or guide me to websites for the employment letter from the employer to apply for my H1 and H4 revalidations at St. Louis. Does anyone know how long it is taking these days?
