H1 Revalidation in US

I have received my H1 extension and need to send my Passport for stamping.

I am in the process of filing my 140/485 concurrently next week.During the same time i will also be sending my passport for stamping.

What do i need to write in the form 156 where it asks If anyone has applied for a nonimmigrant petition on my behalf.

Also in form 157 it says write your name in native language .What does that mean?

Could anyone reply answers to these questions

1) you should write yes, followed by "Employment based immigration filed by <your co.>
2) You can ignore it or no harm in writing the name in your mother tounge.
