H1 in Matamoros - Permit needed?

I am planning to get a H1 visa stamp on my passport in Matamoros. I need some advise on some questions:

Is it needed the services of an attorney? I saw a webpage, saying that they get a permit to cross the border? Is it really necessary that permit?

Also, I have heard that getting a reservation through phone calls is time (and money) consuming. Is it cheaper to set up an appointment via internet?

You are a citizen of _________ ??

Obviously, visa rules depend on that :)
