H1-H4 Visa Revalidation

My wife's H1 was approved last October (2007) through consultant, But she couldn't work because she was pregnant and delivered a baby in November, we have urgent need to travel to India next month and we need to go for visa stamping. she is trying to get a job from last month but nothing is finalized, though couple of cases are very close to get an offer. Now is it anyway possible to save her H1 when we go for visa stamping. she can get maternity approval letter from the consultant but they are not ready to give her any pay slip.
I have a few questions, Please help...
1. Is it certain that if she goes for a H1 stamping without pay slip it will be 100% rejected.
2. If her H1 gets rejected after how many days she can apply for H4. Does she need a new appointment?
3. Is it possible that her H4 also can get rejected because she was in H1 for last 4-5 month and couldn't work.

Please advice what should be the best to do in this situation.
 
I was not asked for paystubs (May 2007) - but that's sheer luck. Please provide more information about how long the H-4 was extended before COS to H-1 was filed. Back in 1999 one of my colleague's B-1 visa was rejected - he was asked to re-apply in 15 days at New Delhi, India embassy. You need to check the consulate website for this.
 
Her H-4 was extended till 2009, I heard there is some thing called COS H4-H1, if she stamp H4 and come back in US can she change her status back to H1?
