H1-B to GC paperwork

I am a current H1-B, who married an american citizen in April. I have been in the country since 1999. We are now living here in the US in North Carolina. I am about to file my Adjustment of Status papers, with the Texas processing center. I am about to file the following forms:
I-485
I-864
I-765
I-130

Am I missing any forms? Do i have to include the medical examination (Form I-693) at this time or can this be done at a later point?
 
I also filed similar forms. I think you missing the form

Form G-325A Biographic Data Sheet
The Beneficiary and Sponsor must fill this.

And, yes you should include

Form I-693 Medical Examination Sheet

The application will be complete with the 325A and yes, you do need the medical exam forms if you are filing at a service center.

Also, you may I-864A if you want to show your income as affidavit of support. As a matter of fact, it is better if you file I-864A along with I-864. Make sure you have the following documents:

Utility Bills
Lease/House papers with both of your names on it (very important)
Tax returns for the last 3 years
Employment letter and pay stubs (for I864 and I864A)
Bank statements
Marriage Certificate
Copy of your passport with visa page
Your H1B documents

I believe thats pretty much it. However, I will go through my paperwork tonight and add more if I have missed something.
