H1-B revaldation returned without any descision

My wife had come to US on Company A visa and worked for that for some time and started working for company B (she got both Visa approvals when she was in India) without filing the Change of employer petition (didn't know to file). After 8 months she came to know that her first employer cancelled the Visa. Then she applied for revaldation and she got all the papers and Passport back without any descision. INS didn't give any explanation or letter saying that the visa revalidation is approved or rejected.

Hi Jaxen/ or any other person, what does that mean? What to do next. Please reply. I appreciate for the reply greatly.
 
So she had only A visa stamp in the passport? By entering using company A visa, but working for company B, she committed a grave mistake. However if it is possible to prove that the start date for company B is within days of entry into US (paystubs, appt. letter), she should be able to get the new visa at a US consulate.
Post dates , if above is not true.

Here is the whole scenario,

She got two independent visa approvals when she was in India and she got stamped her company A visa, came to US and worked for Company A for some time. Then she moved to company B and after a while Company A cancelled her visa.
After 8 months company A cancelled her Visa, we came to know that and we sent her company B visa for revalidation. They sent all the documents back without saying anything.
We had also applied for her H1 with another company and awaiting for the decision.

Jaxen or any other person, can you please reply on this as what is her status now and what is her course of action now.
