H1 B Duration 6 Yrs

On my H1 B
Issue date: Nov 2004
Expiration date : Feb 2007

Total Duration is 2yrs 2 months

My first query is I believe that we get initially visa for 3 yrs,So when it is 2yr 2months does this reduces my 6yrs H1 duration to 5yr 2 months.

My second query is I got my visa stamped on Nov-2004 but came to US in Sep 2005, does the 6Yrs duration start from the time of entering US of from the Issue Date.

I would highly appreciate if I can get some helpful hints from you guys....
