Myself and my wife are Indian planning to send passports for Visa revalidation (presently on H1B and H4 repectively). Are following docs enough for revalidation.
1. DS-156 form for myself and my wife.
2. DS-157 form for myself.
3. One photo for each of us.
4. Orginal passports of both.
5. Fees $100, $50 for each by DD. Total ($300).
6. Employment letter.
7. Orginal H1B and H4 petition. And 194's.
8. Self addressed Fedex bill for return of passports.
Are any other docs required ?
I heard about barcode on DS156. How do I print that doc with barcode. When I print the DS156 form iam just getting “BARCORDE” Bold letters on right side.
In DS 157. In question List of countries visited. I have to write like. Right?
