H1 8th year extension with approved i-140

Dear friends,

I just received my i-140 approved with i-485 pending. (Finally after 4 years of waiting). I am about to extend my H1 visa for the 8th year by myself. Does anyone have a cover letter sample that I can use for my application? and What are the documents I need to extend my H1 visa in my case? (I just want to get some opinions from everybody to make sure that I do not miss any forms)

Here is what I can gather off my head:

For Primary Petitioner
1. I-29 form , H, supplement, I129 H data collection
2. Letter from petitioner
3. LCA ( Form 9035E)
4. I-94
5. All I-797
6. Recent pay stubs
7. Evidence of qualifications
8. Filling fee payable to Department of Homeland Security
9. a copy of visa and passport stamp
10. Proof that you are eligible for 7th yr extension


For spouse
1. I-539
2. copy of dependent’s I-94, I797 and visa stamp

Form G28 is required for using an attorney.

From what I have seen, paystubs are required only when you moved from one company to another. If you are filing extension from the same company, it is not necessary. I did my first 3 year H1 extension and am planning to do my 7th year extension on my own now. I never sent any paystubs the last time and was approved without any problems.
