The Port of Entry and the Date of Entry in the AR 11 form refers to the Last Entry or the First Entry?

I used the date of first entry

i filed AR11 two wks ago, but I used first entry date.

My guess is that the key of AR11 is to get the old and new addresses right. Nothing else matters. Without knowing my filing AR11, my laywer wrote INS a letter (not AR11) informing my ad change.
