Gurus, help with follow to join procedure

Does anyone know how "Follow to Join" procedure works. Here are some questions, I will appreciate it if someone can help.

1) When/where do you will file I-824? What other docs do you need?
2) For "Follow to Join" to work you have to get married before I-485 is filed. If not, what is the cutoff limit?
3) Can you use "Follow to Join" even if you are working on EAD?
