1) Yes, they will take the card back.
2) I checked Permanent Resident - (Not a Commuter). (Didn't think I am a commuter although I didn't understand what exactly it meant.)
3) If you filed i-485 for adjustment of status, it should be your 485 approval date.
4) For me, it's Vermont Service Center.
5) For me, it's obvious just with myself standing there (and probably with my passport and my driver's license indicating my gender). The officer also looked into their computer system to see they actually have my gender alright but they probably goofed up while making the card.
In your case, they need to make sure that your son's middle/initial is in their system. It would be helpful to bring in his passport (and driver's license if he has one). However, if his middle name is not even in their system, you have to show them (I think) a copy of your original I-485 application indicating his middle name was indeed in the application; otherwise, they may not consider it as an administrative error and may charge you the fee for it.
