Hi guys, I got my RFI yellow letter in the mail asking for medical and Biographic information and I have a couple of questions.

1- Where should I send the response? To what address?
2- How many copies of the G-325A should I send?
3- Any recommendation?

I'll appreciate if some of you who already went through this process a couple of months ago and got approval help us. Several forum members will be receiving letters soon.

Thanks

You send it to the address on the upper right corner in the letter which where they sent it from. It is a P.O. Box address..at least that was in my case when I went through it, good luck guys

flashington..Your timeline? Are you going to do another medical?

ND march/01, ,is the first time that I do medicals, I didn't do it when I filed it.

What about the Biographic information how many copies of that form should I send?

The biographic form has 4 pages..all same but all have different footer text. One says applicant then consular..I forgot the rest..

Print out the 4 pages and put a carbon copy behind page 1...or fill all 4 pages..

Someone mentioned you can copy 1st page 3 times..but if you notice, all 4 pages are different.

hi

Download the Bio form from the USCIS site, it is in .pdf format. When you fill it out, it fills out all 4 copies at the sametime. Go ahead and print it out and it will give you 4 copies ready to go. Thats what I did and it works pretty well.

Bio/RFI: 3/05

Four Copies

There are four pages marked at the bottom as follows;
(1) Ident.
(2) Rec. BR.
(3) C
(4) Consulate

G-325 allows you to fill the form on your computer but G-325A doesn't. Looks like we'll have to manually fill out. I don't like using carbon paper. I'll just fill it out four times. Keeping in mind that this is for my GC, I'm willing to fill it out thousand times if I have to.

PM... Did you get the actual notice?

Waiting but not wasting

No, I'm waiting but in the meanwhile, I'm not wasting anytime. I'm filling G-325A as we speak. I have made appointment with my Doctor, who is approved USCIS civil surgeon for tomorrow for TB test and then this Thursday again. Call me crazy but I want to make sure that I have as much info ready as I possibly can before hand. How are you holding up?

Just got it.....

My roommate just called me to let me know that the postman just delivered my RFI. Just as I suspected, they asked for the following;
I-693
I-693A
G-325A
Where is the I-693A form. I don't see it on the USCIS form.
Thanks

Punjabi munda...You may be right about the mass approvals. If that happens, I owe you a lunch in Virginia..if you are close by that is..

I'll come to VA....

Thanks Wantmygcnow, I'll come to VA, just for that lunch as soon as I get a free airfare offer:) I'm from Wisconsin BTW :p

The Doctor's office will fill up that form for you, 693 and 693A. I went there today, the results will be ready in 2 days.

I am still waiting. Maybe I'll do the same, thanks for the update. Congrats on recvng the RFI.

Hang in there buddy...

Thanks Flashington. The reason I asked was, when talking to my Doctor's receptionist today, she told me to bring I-693 form. That's why I thought that I might have to bring I-693A as well which is not on the USCIS web-site.
Free100, did you get your mail today? If not may be you'll still get it today. Otherwise tomorrow depending how far are you physically located from Nebaraska.
Question for everyone else, I used to work for a company before my current employer. That company has relocated to a different address after I left that company. Should I put their current address? The employment letter from that employer when I filed for my I-485 will have their old address. I'm being so careful that I feel like a criminal

Thanks PM. I have asked the Post office to forward the mail to my new address as I moved last month. I already submitted the change of address, but seems like everything at USCIC, it will take some time.
Regarding your question, I think it is better to give them the company's current location. You can put in a remark in Parenthesis "New location".

Free100

Free100 thanks for your free advice ;) . That's exactly what I did. I put a little arrow on the side and wrote (new address). I called my Doctor's office and they do have I-693A at their office. I have already completed all the required immunization shots by the same doctor in the anticipation of this RFI (My insurance covered it :) , sweet). HIV test is done already. Now all he has to do is do the TB test and syphylis, should cost me $150-$170. The rest should be all paperwork since he is my regular doctor. I picked him due to the same reason.
Question for people who have submitted their RFIs. Can I go ahead and send my G-325A today since doctor will mail the test result on his own directly to USCIS? Would I get a copy of those results or not?
Thanks
