Got notice,help needed

Hi,Guys:

I've recived the notice today and they asking the initial evidence which are "form i-693" and "supplemental from to i-693", but there is special massage following:

IF YOU SUBMITTED YOUR FORM I-693 AND/OR THE SUPPLEMENT AT THE TIME OF FILING YOU FORM I-485, PLEAE INCLUUDE A STATEMENT EXPLAINING THIS WHEN YOU SUBMITTING YOUR RESPONSE TO THIS REQUEST.HOWEVE, BE ADVISED THAT IF THESE DOCUMENTS ARE NOT PRESENT WITH YOUR APPLICATION AND YOU FAIL TO SUBMIT THEM IN RESPONSE TO THIS REQUEST YOUR APPLICATION MAY BE DENIED.

Is there any of your guys can help me to understand this, what i should do?
Should i just provide the explaining letter or i should provide the form i-693 again.

Hi everyone.
Yes I receive a samething RFI a yellow notice regarding a medical record I 693 and supplemantal form plus G-325 A the BCIS said your response must be received in this office by 15 September 2005 your case is being held pending your response. But I do have a lawyer who this ready so i will call next week if he receive the same notice, and also something weid i never done any FP.

Hi Espritis
When I had check online just a days has been changed to 06/23/2005, but never mention the RFI has been sent to me and 2 days later, I had receive yellow notice in the mail.
