Goods to Follow list

Does anyone who already landed in Canada, could you please post your sample template for 'Goods to follow list'.
Can we create one in excel document and carry it with us? if so what are the details we have to mention in that sheet?

Can we fill it already and take it with us or have to fill a new goods to follow list at the port of entry to Canada?
