RFE will be issued by an officer, as far as I understand. RFE could be for anything related to the application like signature, photos etc. In other cases, company name change or merger also end up being RFE potentials. Other common reason is like mine i.e. RFE for last 3 years W2s and most recent 4 paystubs.

Ravi, when did u get ur RFE, what is ur ND, AP, EAD dates. Did u talk to IIO?

Here are my details-
EB3 RIR India
PD - May 98 (yes, May 98)
RD/ND - Oct 99
1st FP - Jul 2000
2nd FP - Jan 2001
May 1st : My priority date became current
May 3rd : RFE issued for last 3 years W2s, 1040s and most recent 4 paystubs
May 11th : RFE replied by USPS Express Mail
May 12th : The docs delivered to INS by USPS as per record from USPS.com
And till now, no updates in AVM even for the receipt of RFE
Talked to IIO this week, she didn\'t tell anything new other than they haven\'t recd the RFE reply from me. I have the signed receipt dated May 12 that the package was delivered.
Waiting, waiting, and waiting !

Please ask your lawyer to send the documents again to INS. Looks like your case has taken too long time. Some times they would have missed it also, I have seen some people saying that they had also Faxed the documents. Please do it before your end date, I would say do it tomorrow.

I tried my best with my lawyer- he has just agreed to send inquiry fax. He does not want to send the docs again. I am hoping that if he sends the fax, I will atleast have two documents as proof- one the return receipt and second the fax confirmation. That is all I can hope to get out of him, for now. Thanks for your advice. I will try again with him tomorrow.
