General Question on AR-11 Change of Address form

My wife works for company a beauty shop at Maryland. The Headquarter of the company is in Texas from where her paystubs will be generated.
In the chage of address form AR-11 , should I put the Maryland Address or the address of the company's head quarters based in TX.

Since she is working on EAD, what should I put her status in the country :

H-4 or EAD or I-485 ( adjustment of status )

In case if I put EAD or I-485 what will be the duartion of legal stay in the country?

I wanted to add one more point that her W2 will have the texas address.

Maryland Address should be fine. CIS usually doesn't care about the working address as long as they have your new home address...
